Teacher-coaches and Athletic Directors complete all OFSAA paperwork
If you are not familiar with the new OFSAA HUB system, please ask the Athletic Director at your school to assist.
The following form must be completed by all schools and hard copies given to the regional convenor at the the regional championships. Regional convenors will courier copies to the OFSAA hosts by Monday, June 2 at 12:00pm.
Online OFSAA HUB Registration – DUE SATURDAY MAY 31 AT 11:59PM
Athletic Directors must start the registration process by completing & uploading the following mandatory forms through the OFSAA HUB:
* If you have multiple age categories attending OFSAA – You can create a “Senior/Varsity” / Gender: “All-Gender” Team on the Hub.
Creating one coed team versus male and female teams will not affect race entry, but will cut down on your scanning and uploading
Remaining Mandatory Forms: DUE TUESDAY JUNE 3 @ 3:30 PM
Athletic Directors/Coaches must submit and upload the remaining mandatory forms with appropriate signatures:
- Student – Waiver of understanding (One form per student. All forms should be scanned into one file)
- Coach – Confirmation and Release (One form per coach. All forms should be scanned into one file)

FORMS TO BE DISTRIBUTED TO STUDENT-ATHLETES (IT IS RECOMMENDED THAT COMPLETED FORMS ARE COLLECTED BY COACHES):
- OFSAA Rules of Behaviour
- Student-Athlete/Parent Concussion Review Confirmation– Confirmation de l’examen de la commotion d’athlète/parent
- Participation Informed Consent and Assumption of Risk (Under 18)
- Photo Release Form
- Adult Release of Liability (18+) (If no one is over the age of 18 and you do not need to distribute)

ROWANS LAW:
As mandated by Rowan’s Law, every student-athlete, coach and trainer is required to confirm they have reviewed the Concussion Awareness Resource and the OFSAA Concussion Code of Conduct. In order to participate in this event it is mandatory that a confirmation form for each participant be completed and submitted with registration.
