Championship Registration Steps:

 

1. Online OFSAA HUB Registration – DUE THURSDAY, MARCH 6 @ 11:59PM

Athletic Directors must start the registration process by completing and uploading their eligibility sheet through the OFSAA HUB and completing the team/season information form linked below:

  • Printed & Signed HUB Eligibility Roster (example)
    Note: As per the OFSAA Playing Regulations for hockey, only 25 participants per team (combination of players and team personnel) can register. Only these 25 individuals should appear on the eligibility sheet (you may cross off names by hand if needed).
  • Team & Season Information Form
    Click the linked title above to complete the team & season information form that is required for seeding. This form is new this year and this portion of the registration process is to be completed through this link. You are not required to upload this form to the OFSAA HUB – your responses will be collected through the link following your submission. Please complete this form with as much detail as possible as it greatly assists in the seeding process.

 

2. Remaining Mandatory Forms – DUE MONDAY, MARCH 17 @ NOON

Athletic Directors/Coaches must submit and upload the remaining mandatory forms with appropriate signatures through the OFSAA HUB:

 

3. On-Site Registration

On-site registration will take place at St. Maximilian Kolbe CHS (cafeteria) between 3:00 pm and 8:30 pm. Please bring three (3) separate cheques for the following fees:

Registration Information:

Location:  St. Maximilian Kolbe CHS – 278 Wellington St East – Aurora, ON – L4G 1J5 (registration will take place in the cafeteria)

Time: Monday March 17, 2025 – 3:00 pm – 8:30 pm

Reminders:

  1. We ask teams not to show up earlier than 3 pm as we need to wait for our school busses to leave the bus loop prior to accepting OFSAA team busses.
  2. Upon arrival, busses will drop team members off in our bus loop which is located on the west side of our building, off of Industrial Parkway, north of Wellington Street East.
  3. Team members will enter the school through the bus loop main entrance, where OFSAA volunteers will usher them to the cafeteria.
  4. Once you are in the cafeteria, coaches will check in with one of the convenors and the following will happen:
  1. Please ensure you bring all 3, separate cheques (OFSAA Entrance Fees, Banquet Fees and OFSAA Fees).  Those will be handed in at registration, and you will then be given a coach’s package and your team OFSAA banner.
  2. From there, your team will be ushered to the place where you will be taking your official OFSAA team photo (please ensure your players enter the cafeteria wearing what they will need to wear for your official photo as there will be no dressing rooms to change).
  3. After your team photo, each player will receive a gift bag that contains various items.
  4. After you have picked up your player gift bags, your last stop will be to our official OFSAA clothing supplier – Canuck Stuff.  At that point, coaches will be given their team pre-orders that were ordered online.  Also, players and coaches can purchase additional OFSAA apparel if they did not have a chance to purchase it during the pre-order period.
  5. Once your apparel needs have been taken care of, teams will exit the cafeteria doors closest to the bus loop to board their bus.

 


Items to Review:

The following forms do not need to be uploaded but they are to be distributed to participants. It is recommended that the completed forms are collected and kept on file by coaches. 
Rowan’s Law:

As mandated by Rowan’s Law, every student-athlete, coach and trainer is required to confirm they have reviewed the Concussion Awareness Resource and the OFSAA Concussion Code of Conduct.  In order to participate in this event it is mandatory that a confirmation form for each participant be completed and submitted with registration.
