Registration Forms and Fees

1. Online OFSAA HUB Registration – DUE TUESDAY, FEBRUARY 18 AT 11:59PM

 Athletic Directors must start the registration process by completing & uploading the following mandatory forms through the OFSAA HUB:

2.  Remaining Mandatory Forms – DUE THURSDAY, FEBRUARY 20 AT 3:30PM

Athletic Directors/Coaches must submit and upload the remaining mandatory forms with appropriate signatures to the OFSAA HUB:

3. Championship Fees

Submit on Sun February 23, 2025 from 1:30pm to 3:30pm at the OFSAA Alpine Championship Registration Desk, Osler Bluff Ski Club. (Ski Break Room, Main Club House)

  • Each school (one coach) must bring four (4) cheques to Registration
    • one for the total of all Osler Bluff Race Entry fees written to Osler Bluff Ski Club
    • one for the total of the OFSAA Athlete Entry fees written to Our Lady of the Bay CHS
    • one for the Bib Deposit Fee written to Our Lady of the Bay CHS
    • one for the total of all Banquet fees written to Our Lady Bay CHS
  • Each Association will send a cheque, with one of the schools, for the total of all OFSAA Participation fees for that entire association written to OFSAA

***Coaches and student athletes will be able to ski on Sun Feb 23, 2025, in the afternoon, for $25/person plus taxes. Please go to the Race Office to purchase this ticket.***


Items to Review:

The following forms do not need to be uploaded but they are to be distributed to participants. It is recommended that the completed forms are collected and kept on file by coaches. 
Rowan’s Law:

As mandated by Rowan’s Law, every student-athlete, coach and trainer is required to confirm they have reviewed the Concussion Awareness Resource and the OFSAA Concussion Code of Conduct.  In order to participate in this event it is mandatory that a confirmation form for each participant be completed and submitted with registration.
