Championship Registration Steps:

 

1. Online OFSAA HUB Registration – DUE SATURDAY, NOVEMBER 2 @ 11:59PM

Athletic Directors must start the registration process by completing and uploading their eligibility sheet through the OFSAA HUB and completing the team/season information form linked below:

  • Printed & Signed HUB Eligibility Roster (example)
    Note: As per the OFSAA Playing Regulations for field hockey, only 25 participants per team (combination of players and team personnel) can register. Only these 25 individuals should appear on the eligibility sheet (you may cross off names by hand if needed).
  • Team & Season Information Form
    Click the linked title above to complete the team & season information form that is required for seeding. This form is new this year and this portion of the registration process is to be completed through this link. You are not required to upload this form to the OFSAA HUB – your responses will be collected through the link following your submission. Please complete this form with as much detail as possible as it greatly assists in the seeding process.

 

2. Remaining Mandatory Forms – DUE TUESDAY, NOVEMBER 5 @ 3:30PM

Athletic Directors/Coaches must submit and upload the remaining mandatory forms with appropriate signatures through the OFSAA HUB:

 

3. On-Site Registration:

On-site registration will take place at the Tournament Central Tent on Thursday, November 7th between 8:00am and 1:00pm. More details will be shared at the virtual coaches meeting.  Please bring three (3) separate cheques for the following fees:

OFSAA Participant Fee (Per Team) $ 57  OFSAA
Banquet Fee (Per Participant) *this number should reflect those on your eligibility sheet exactly $ 40 Ridley College

 


Items to Review:

The following forms do not need to be uploaded but they are to be distributed to participants. It is recommended that the completed forms are collected and kept on file by coaches. 
Rowan’s Law:

As mandated by Rowan’s Law, every student-athlete, coach and trainer is required to confirm they have reviewed the Concussion Awareness Resource and the OFSAA Concussion Code of Conduct.  In order to participate in this event it is mandatory that a confirmation form for each participant be completed and submitted with registration.
