Association Commitment Form

Associations withdrawing entry commitments after the December 1st deadline will be subject to penalties outlined in the OFSAA Constitution.

Please make sure that your member schools are aware of the possible ramifications of withdrawal from a championship BEFORE submitting this commitment form to the OFSAA.

The deadline for all Winter sport commitments is December 1st, 2024.

Note: This form is not intended to be sent to individual schools.
