2025 Ontario Volunteer Service Award Nomination Form
OFSAA nominates 6 individuals, 3 males and 3 females, who are current or retired teachers or administrators with a minimum of 10 years of teaching, for the Ontario Volunteer Service Awards. This award recognizes volunteers for providing committed and dedicated service to an organization. OFSAA nominates the volunteers that are responsible for the conduct and success of school sport at the local, association and OFSAA levels. To receive this award, there must have been involvement at the OFSAA level whether it is/was a member of one of the committees, as a representative, or as a convenor or member of a convening committee of OFSAA events. There are deserving individuals in all associations who are worthy of this recognition and we hope that you can provide the names of at least one or two males and females that may be considered. Following your submissions, staff will select the nominees and put forward the application.
